Request For Documents Policy

In an effort to streamline clinical and administrative services, effective November 2022, BICAMH introduced its Report Request Form. 


Request for reports must be made formally via email, and a request form (provided by BICAMH) must be submitted to the office manager.


Forms that can be requested include the following: 

  • Past Assessment Reports
  • Behavioural & Emotional Assessment Reports
  • Proof of visitation etc. 


For a child(ren), a request can only be made by the parent(s) or legal guardian(s). If an adult, the request will only be accepted from the client on file or an authorized individual. To maintain our standard of confidentiality, only requests submitted by the client or an authorized individual will be honoured.


Reports that date more than three years will include a disclaimer stating results no longer reflect the current functioning of the client. In these cases, re-testing is strongly recommended.


If the request for the report has been approved, you will be notified by our Administrative Assistant. You will be given the option to receive via email or collect from our #110 Collins Avenue location, with respective ID. Turn-around time for approved report requests is 48 hours, given clinician availability.
